In short

What is Billing Exception Management?

It is the work of clearing the payments and notices that do not apply themselves: unapplied cash with no policy number, returned debits and checks, premium finance company notices and refunds waiting on approval. Billing Exception Management puts all four on one board, ranked by the cancellation each one could cause, with agents proposing the match and the action.

The problem

Why a customer who paid still gets cancelled

A direct-bill carrier takes in thousands of payments a day — lockbox checks, ACH debits, portal and phone payments, wires, mortgagee escrow checks, premium finance fundings. Most apply themselves. The rest land in separate work queues: a check with no coupon and no policy number, a debit back as R02 “account closed”, a finance company’s request for cancellation, a return premium refund waiting on approval.

Each queue is worked by hand, oldest first, and nothing ranks the items by what they will cause. Matching on amount fails because installments are standard amounts — at Kestrel Point Mutual this morning, four open installments are exactly $1,840.00. The answer is written on the check, in the memo line, and nobody has time to read it. Meanwhile the delinquency process runs on, the notice of cancellation for nonpayment takes effect, and the agent gets the call from a customer who mailed the check a week ago.

One case, step by step

One Monday morning, from lockbox file to daily report

Kestrel Point Mutual, direct bill, Monday Oct 12 at 09:40. Here is the morning, screen by screen, in the working solution.

  1. 0109:40

Every exception on one board, ranked by what it could cause

Renee Castillo · Cash application specialist

Renee opens the board. Today 2,422 payments came in, worth $2,609,396.13 — 2,414 applied straight through. What is left sits in four lanes: unapplied cash, returned payments, premium finance notices and refunds awaiting approval, each item showing the money, who paid, the likely policy and the next step.

“Kestrel Point Mutual · direct bill · ranked by the cancellation each item could cause”

  2. 0206:40

Two customers who paid, about to be cancelled

    Cancellation clock keeper

At 06:40 the clock keeper lined up every notice of cancellation for nonpayment against money received, retries and cures. Of 214 cancellations taking effect in the next 14 days, two are on policies already paid: Ridgeback Paving Co., CA-6120-0094, effective Oct 13, and Marisol Delgado, HO-4471-0938, effective Oct 16 — her $1,840.00 check is sitting in unapplied cash.

It never rescinds a notice by itself — it flags “paid but cancelling” to the person who decides.

  3. 03One click

The check is read, not just its amount

    Remittance reader

UC-0412-017: lockbox check #4127 for $1,840.00 from Rafael Reyes, Ferrous Valley Credit Union — no coupon, no policy number in the remittance file. The reader reads the check image front and back: the payer block names Rafael Reyes and Marisol Delgado at 7 Larkspur Ct, and the handwritten memo line reads “Delgado HO 4471-0938 Oct”, at 96 %.

By amount alone: 4 matches, best 25 %. By the check itself: 97 %.

  4. 04Next

Every candidate compared, field by field

    Payment matcher

The full record puts the check image beside the policy’s billing schedule: quarterly, $7,360.00 a year, installment 2 of 4 for $1,840.00 due Oct 1 and unpaid, notice of cancellation mailed Oct 2. HO-4471-0938 scores 97 % — memo, payer, address and amount agree. Teodoro Reyes, same surname and the same amount due Oct 8, scores 2 %.

“The policy has a notice of cancellation pending, so a person confirms.”

  5. 05Confirmed · 09:41

    Applied, and the notice rescinded

Renee Castillo · Cash application specialist

Renee confirms. $1,840.00 is applied to installment 2 of 4, the notice of cancellation effective Oct 16 is rescinded, a “payment received, your policy stays in force” letter is queued to Marisol Delgado and Rafael Reyes, and Lindqvist Insurance Agency gets a note. The item leaves the lane, Oct 16 on the calendar turns green and the count drops to 1 already paid.

  6. 0607:58 email

A finance company withdraws its request — the day before

Hollis Tran · Premium finance coordinator

Summit Ledger Premium Finance writes that the default on agreement SLF-20977 has been cured: $1,486.20 received Oct 8, and its request for cancellation dated Sep 28 is withdrawn. The cancellation of Ridgeback Paving’s commercial auto policy is still on file, effective Oct 13 at 12:01 a.m. The premium finance reader proposes rescinding it; Hollis decides.

“The insured paid the finance company; it withdraws its request — the cancellation takes effect tomorrow unless rescinded.”

  7. 07Next

One request to cancel is lawful. One is too early.

    Premium finance reader

Granitebay Premium Funding asks to cancel Corliss Bakery’s business owners policy effective Oct 14. Notice of intent was mailed Sep 22 — 17 days, against 10 + 3 required — so the reader works out the unearned premium, 182 of 365 days of $4,780.00 = $2,383.45, and splits it: $1,106.40 to Granitebay up to the unpaid balance, $1,277.05 to Corliss Bakery, due by Nov 13. Altamere Capital’s request on Bayline Roofing gave only 6 days, so that one is held and Altamere is told the earliest lawful date is Oct 19.

It never cancels or reinstates a policy — it proposes, and holds any request with a short notice period.

  8. 08Returns file

Every returned debit gets the action its code allows

    Returns handler

Seven returns, $5,393.80. Harriet Vance’s R01 is the third insufficient-funds return, so no retry is left and a nonpayment notice is due; Keisha Monroe’s first R01 is retried on Oct 15; Gerald Whitcombe’s R02 needs a new payment method; Tidewell Logistics’ R29 stops all debits until a new authorization. The 60-day unauthorized rate stands at 0.08 % of the 0.5 % Nacha allows.

Only R01 and R09 are re-presented, at most twice, with “RETRY PYMT”.

  9. 09Refunds

Refunds routed to the right payee and the right approver

Marcus Webb · Billing team lead

Five refunds wait, $8,284.75. Sablewood Antiques’ $4,118.75 return premium — pro rata, 200 of 365 days — and a $2,906.00 two-party check to Nadia & Leon Ferraro and Halcyon Home Loans for a roof credit endorsement are over $2,500, so they go to Marcus. A $214.00 credit dormant 34 months for Curtis Langley is on the unclaimed property watch, reportable Dec 2026.

The refund preparer never releases money; over $2,500 goes to the team lead.

  10. 10Daily report

The daily exceptions report, from the audit trail

    Adaeze Nwosu · Finance controller

27 items on the board this morning, 15 cleared, $13,651.88 of unapplied cash still open across 8 payments, and 1 of 214 cancellations still on a paid policy. Every decision is listed with who made it — Renee’s 09:41 application, the payment matcher’s proposal, the premium finance reader’s hold — and open items carry their reason and owner into tomorrow’s board.

Built on the engine

6 agents. Each with one job, and hard limits.

Six agents read every payment, return and notice — check images included — and propose the action; a billing specialist confirms.

Remittance reader

Reads lockbox files, check images front and back — payer block, amount, memo line, MICR line, endorsement — coupons, ACH addenda, wire references, escrow and finance-company remittances.

  • Never changes a payment record
  • Quotes the image region for every value it reads
  • Gives a reading confidence for every value
Payment matcher

Finds candidate accounts, policies and installments from memo, stub, payer, address, mortgagee loan number, finance agreement and amount, scores each, and shows the amount-only result beside the full result.

  • Never applies below 95 % without a person
  • Never applies to a cancelled policy without a person
  • Never splits a payment across insureds
Returns handler

Reads the returned payment file, reverses each payment, sets the action its reason code allows and schedules R01 and R09 retries; drafts the nonpayment notice when no retry is left.

  • Re-presents only R01 and R09, never more than twice
  • Tracks the 60-day return rates against Nacha thresholds
Premium finance reader

Reads notices of intent to cancel, requests for cancellation, reinstatements and requests for return premium; matches the agreement to the policy, checks 10 + 3 days’ notice and splits the unearned premium.

  • Never cancels or reinstates — proposes
  • Holds any request with a short notice period
Cancellation clock keeper

Every morning lines up every pending nonpayment cancellation against unapplied cash, reinstatement notices and scheduled retries, and flags “paid but cancelling”.

  • Never rescinds a notice by itself
Refund preparer

Prepares refunds and return premium with a worksheet; checks the payee — finance company up to its unpaid balance, two-party check when a mortgagee is on the policy, otherwise the insured — and watches dormant credits.

  • Never releases money
  • Over $2,500 goes to the team lead
Billing specialist

Confirms; team lead approves over $2,500. The agents propose; a named person decides.

Ask in plain words

Ask about any payment, in plain words

Ask about any payment, return, notice or refund — or tell it what to change. Every answer points to its source.

Why is the $1,840 check matched to the Delgado policy?

Because the check says so. The remittance file had no policy number, and four open installments are exactly $1,840.00, so the amount alone is a guess. The memo line reads “Delgado HO 4471-0938 Oct”; the payer, Rafael Reyes, is a named insured on HO-4471-0938 at the same address; and installment 2 of 4 for $1,840.00 is unpaid. The notice of cancellation takes effect Oct 16.

Which customers who paid are about to be cancelled?

Two: Ridgeback Paving Co., CA-6120-0094, cancels tomorrow — Summit Ledger withdrew its request. Marisol Delgado, HO-4471-0938, cancels Oct 16 — her $1,840.00 check is in unapplied cash. The other 212 cancellations in the next 14 days have no payment received and no cure pending.

Are we near the Nacha limit on unauthorized returns?

No. Over the last 60 days 33 of 41,280 debits came back unauthorized — 0.08 % against the 0.5 % threshold. Today adds two (R10 Lena Farrow, R29 Tidewell Logistics); both have debits stopped until a new authorization. Administrative returns are 0.61 % of 3 %.

Hold any finance company request with less than 13 days’ notice

Done. New rule: “Request for cancellation with fewer than 10 + 3 days since the notice of intent → hold and write to the finance company.” It already applies to Bayline Roofing’s request (6 days). You can change it in Settings.

Questions

What billing teams ask us.

What is billing exception management in insurance?

It is the work of clearing the payments and notices that do not apply themselves: unapplied cash with no policy number, returned debits and checks, premium finance company notices and refunds waiting on approval. Billing Exception Management puts all four on one board, ranked by the cancellation each one could cause, with agents proposing the match and the action.

How does it match unapplied cash with no policy number?

The remittance reader reads the check image itself — payer block, memo line, MICR line, endorsement — and the payment matcher scores every candidate on policy number, payer against named insureds, address, loan or agreement number and amount. The amount-only result is shown beside it, so you can see why the match was chosen.

Does it stop a cancellation on a customer who already paid?

Each morning the cancellation clock keeper lines up every pending nonpayment cancellation against money received, reinstatement notices and retries, and flags any that are paid but cancelling. It never rescinds a notice itself; a person confirms the rescission from the board.

How does it handle premium finance notices?

The premium finance reader reads each notice, matches the agreement to the policy and checks 10 + 3 days since the notice of intent before a request for cancellation is acted on. It works out the unearned premium and its split — the finance company up to the unpaid balance, the rest to the insured — and holds any request with a short notice period.

How are returned payments worked?

The returns handler reverses each return and sets the action its reason code allows: R01 and R09 are retried after 3 business days, at most twice; R02, R03 and R04 need a new payment method; R05, R07, R10, R11, R29 and R51 stop all debits until a new authorization. It tracks the 60-day return rates against the Nacha thresholds.

Do people stay in control of the money?

Yes. Agents read, match and propose; they never apply below the auto-apply threshold, cancel or reinstate a policy, or release money. A billing specialist releases refunds up to $2,500, the team lead approves up to $25,000 and the finance controller above, and every decision is in the audit trail.

Can we set our own thresholds and state rules?

Yes. The auto-apply threshold, check image reading, write-off tolerance, refund authority, retry timing, nonpayment and premium finance notice days, return premium deadlines by state and the dormant credit letter are all settings, and you can add rules in plain words.

How long does it take to go live?

The Agentic Solution Engine builds and deploys it from your requirements and documents — your billing rules, state notice days, authority limits and a sample of lockbox, returns and finance company files — and it goes live once every quality gate has passed. We will walk you through it on your own exceptions first.
